Do I Qualify?

✓ CDPAP clients must be eligible for Medicaid

✓ CDPAP clients must have a chronic or ongoing medical condition

✓ CDPAP clients must require ongoing assistance with the activities of daily living (ADLs)

✓ CDPAP clients must be self-directing or have a designated representative who is willing to direct the client's care per CDPAP's requirements

✓ CDPAP-qualifying caregivers can be a child, sibling, parent, close family member or friend of client

✓ Client's spouse cannot qualify as a CDPAP caregiver

✓ Caregiver must be legally allowed to work in the United States

✓ Caregiver must be able to regularly provide skilled care and custodial services for client
